I have a family plan with 3 phones, I also have the need to add a phone +
number to this plan. Given that the phone is E911 compliant, can this be
done online? Or does adding a tel # to the plan require a shop / store visit
    ?

First you'll need to add a line to your account. You could do that online,
in person at a store or by phone with the customer no-service people. You
can activate the new phone online --assuming it's VZW-compliant.

  3. Like doug says you can do this with those 3 options. Just be careful
to make sure they don't try to change your entire plan to add that one
line. They may say that "you have an older plan, and we can't add this
line to your account without putting you on a new plan".

Just be careful with the contract stuff cell phones are getting sneaky
    with that.

I wenty to the store because online wouldn't allow me to complete, without
changing to a "700 minute" plan..

Well, as warned, The 500Min plan was not available in store either.
Adding a phone would have required the following charges.

    1: upgrade to 700Mins. @ $ 10.00 pm
    2: add phone + number @ $ 10.00 pm
    3: 1 time $ 25.00 transaction fee..
4: 1 time $ 40.00 move new # to old phone, add old # to new "free"
    Samsung SCH-A870 phone.

After creating a "small in-store scene" and threatening a "large scene",
    They waived the 2 fees..
I felt like a Dentist, pulling teeth. You'd think the money was coming out
    of their pockets...
    LOL

This sounds a lot like the "Verizon takes long term customers hostage"
thread. Bet you now have all family plan phones under new contracts with
fresh $175 early termination fees each.
